There are different types of bipolar disorder, but all involve some combination of depressive and manic or hypomanic episodes.

In this editorial, we review the evidence that addictions are a key challenge in bipolar disorder, examine putative neurobiological mechanisms, and reflect on the limited clinical trial evidence base with suggestions for treatment strategies and further developments. Ondansetron is a 5-HT3 receptor antagonist used to prevent nausea and vomiting caused by chemo- or radiation therapy. A controlled study suggested a reduction of alcohol consumption with ondansetron (126).

Why are addictions so prevalent in bipolar disorder?

Looking at specific countries, a representative survey applying the Composite International Diagnostic Interview CIDI (3) for ICD 10 and DSM-IV criteria reports a 1-year prevalence rate of 1% for BD -I and 0.6% for BD-II disorder for Germany (4). The same study reports on a 1-year prevalence of 5.7% for substance abuse (except nicotine) according to DSM-IV criteria. Three percent fulfilled criteria for alcohol dependence and 1.8% for abuse (4). In a prior survey, looking at lifetime prevalence rate, the same group reports on similar numbers for BD, and 9.9 and 8.5% for alcohol abuse and dependence, respectively (5). These numbers are in a similar range as in other European countries; while prevalence rates from the US are much higher, both for BD and substance abuse/dependence (6).

What is a substance use disorder?

All of them complete measures of mood symptoms, life functioning, alcohol use and more every two months throughout their involvement in the study. For example, having a family member with an alcohol use disorder is a risk factor for both depression and alcohol use disorder. Emerging research has found that there is a genetic link between AUD and depression.

Conversely, thoughts and behaviors that may increase the risk of relapse to one disorder will similarly elevate their chances of relapse to the other disorder. During therapy, you can learn coping mechanisms that can help you return to life without drinking. The good news is that treating both alcohol misuse and depression can make both conditions better. Alcohol use disorder and depression are two conditions that often occur together. What’s more, one can make the other worse in a cycle that’s pervasive and problematic if not addressed and treated.

  1. Cyclothymia is a disorder in the bipolar spectrum that is characterized by frequent low-level mood fluctuations that range from hypomania to low-level depression, with symptoms existing for at least 2 years (American Psychiatric Association APA 1994).
  2. While both bipolar disorder and SUDs have established treatment approaches, further study is needed on how to best treat both conditions together.
  3. Children who have major depression as a child may drink earlier in life, according to one study.
  4. Alcohol use disorder and depression are two conditions that often occur together.
  5. Sperry notes that previous studies have shown that more than half of people who have a bipolar disorder diagnosis also experience alcohol use disorders sometime in their lives, and that many report using alcohol to help them get to sleep.

In these, you can also find support from others in the same situation. However, for the best results, your doctor will likely treat them together. Findings may aid patients and their clinicians to have conversations about abstaining from alcohol vs. engaging in harm reduction strategies, Sperry notes. For instance, patients who see alcohol as a tool to get to sleep or calm anxiety may be best off focusing on keeping their alcohol use low and stable, and avoid bingeing. This was true for individuals with both of the most common forms of the condition, called bipolar I disorder and bipolar II disorder, although it was even more pronounced in individuals with bipolar II disorder.
